Many of our patients have had pain for many years prior to their first visit. They usually arrive with stacks of reports, MRIs and x-rays. The reports’ diagnoses are typical – several herniated discs, spinal stenosis, foraminal stenosis, or osteoarthritis. The patient’s fascial system is never mentioned. This is a shame because abnormal changes in fascia can invoke pain.

According to Human Anatomy and Physiology, fascia a structure of connective tissue that surrounds muscles, groups of muscles, blood vessels, and nerves, binding some structures together, while permitting others to slide smoothly over each other. The fascial system surrounds and holds all of our organs, nerves, blood vessels, and skin together.

When we move, the skin, muscles, blood vessels, nerves, etc. should easily slide over each other with minimum effort. And this is where the problem exists. In people suffering from muscle/skeletal pain, these tissues do not easily slide. They become caught, sticky and hook on to one another. The thickening, increases friction invites further inflammation, reduces mobility, reduces healing, and increases pain.

The recipe for reducing fascial thickening is compression and increased temperature or friction. This many be the reason why deep tissue massage often brings relief.

The chiropractic adjustment often reduces fascial thickening and when combined with soft tissue techniques such as the Graston Technique, our fascia becomes more pliable, supple, and less inflamed.

To keep these tissues supple, treatment plans should also include further adjustments, stretches, exercises and a way to assess faulty movement patterns that are often the true cause of pain such as the Functional Movement Screen.

As research progresses, we discover that muscle/joint pain origins are not only restricted to our joints, other structures such as our fascia play a big role.    If ignored, our pain and impairment often goes on.

The best athletes are a marvel to watch.  Their movements are flowing and their form is flawless.  But for the rest of us whose limbs do not move evenly and whose neck moves more one way than the other, the problem may be due to a  stiff spine.  This is where the benefits of chiropractic are immediately noticeable.

The 24 movable vertebrae are involved in all bodily movements.  For example, the spine stabilizes our foundation or contributes to the actual motion of a volley ball player’s spike, a pitcher’s pitch, a bowlers throw, a golfer’s swing, and a runner’s gait.

When a portion of the spine becomes stiff through injury, arthritis, illness, or trauma, the body compensates by changing how our body moves.  Previously learned athletic movements are changed and performance suffers.  This compensation protects the spine but unfortunately decreases athletic performance.

The key to restoring performance is to unlock the spine (decreasing its stiffness) through the chiropractic adjustment.  The adjustment breaks up spinal adhesions which when broken immediately allows the spine to move more.  The spine is now better able to participate with other limbs in athletics.  Performance and form then improve.

Trigger points and muscle soreness are not as common in animals as they are in people.  Trigger points most commonly occur when we move in ways that significantly differ than how our prehistoric ancestors moved.  Our body which evolved over many hundred millennia adapts best to a prehistoric world.   Trigger points result when our movements are not “prehistoric movements” but modern movements filled with repetitive tasks of tiny motions, such as sitting and typing.

Since a dog’s life is filled with movements that match its genetic heritage, dogs do not get trigger points.  However, since modern dogs do not move as much as their prehistoric brethren, modern dogs will gain weight and become more sluggish.

 A trigger point complaint is a common reason for first consulting a chiropractor.  All chiropractic care plans should include life style recommendations and ergonomic advice to minimize the toxic effects of possessing a prehistoric body in a modern world.

A common strategy to reduce neck pain is to wear a soft cervical collar.  The thought is that by immobilizing or by resting the neck, the pain will naturally decrease pain and healing will improve.  This thought is NOT TRUE and can delay healing.   The primary purpose for wearing a any cervical collar is to protect the spinal cord from injury, not to reduce pain.  Research has already shown that wearing a soft cervical collar prescribed for whiplash injury has been shown to be clinically ineffective. 

 A study published in the Journal of Manipulative and Physiological Therapeutics, by Katherine Whitcroft, BSc, ET AL., showed that even wearing a soft cervical collar did not adequately immobilize the spine to protect the spinal cord from potential damage.  A rigid cervical brace proved effective in restricting cervical range of motion and should be worn if cervical instability is remotely suspected. 

Wearing a soft cervical collar has other ramifications.  By partially immobilizing the neck:

  • Adhesions develop more quickly,
  • Muscles, ligaments, tendons, fascia, and joint capsules shorten,
  • Fluid accumulates,
  • Muscles atrophy,
  • And cervical joint surfaces become duller and rougher.

A soft cervical collar simply should not be worn.  It does not improve healing and if the spinal cord is suspected of being or becoming injured then a rigid cervical brace should be worn and medical treatment should obtained.

There is a growing body of evidence that wellness care provided by doctors of chiropractic may reduce health care costs, improve health behaviors, and enhance patient perceived quality of life.  Until recently, however, little was known about how chiropractic adjustments affected the chemistry of biological processes on a cellular level.

In a landmark study published in the Journal of Vertebral Subluxation Research (JVSR), chiropractors collaborating with researchers at the University of Lund found that chiropractic care could influence basic physiological processes affecting oxidative stress and DNA repair.  These findings offer a scientific explanation for the positive health benefits reported by patients receiving chiropractic care.

Dr. Christopher Kent, one of the authors explained, “Going through life, we experience physical, chemical, and emotional stress.  These stresses greatly affect the function of the nervous system.  We hypothesized that these disturbances in nerve function could affect oxidative stress and DNA repair on a cellular level.”

“Oxidative stress, metabolically generating free radicals, is now a broadly accepted theory of how we age and develop disease,” Kent continued.  “Oxidative stress results in DNA damage, and inhibits DNA repair.  DNA repair is the mechanism which fixes the damage caused by environmental impact.”

Chiropractors apply spinal adjustments to correct disturbances of nerve function, which adversely affect cellular DNA repair.   “Chiropractic care appears to improve the ability of the body to adapt to stress,” continued Kent.

The researchers measured serum thiol levels in

  • 21 patients under short-term chiropractic care,
  • 25 patients under long-term chiropractic care.
  • 30 non-chiropractic subjects.

Chiropractic was found to significantly increase serum-thiol levels compared with the non-chiropractic subjects.

Serum thiols are primary antioxidants, and serve as a measure of human health status.  The test provides a surrogate estimate of DNA repair enzyme activity, which has been shown to correlate with lifespan and aging.

This study demonstrated a strong correlation between structure, nervous system function and oxidative stress.  Improving structure through chiropractic yields positive benefits far beyond back pain – its benefits extend to lifespan and aging.

There is a common myth perpetuated by medical doctors and orthopedic surgeons that herniated or degenerated discs are the primary cause of back pain.  Spinal Surgeons can study a lumbar MRI, point to a degenerated disc, and claim that the disc is the cause of the pain and surgery is the only remedy preventing possible paralysis.  But in the 1990’s Scott Boden, MD and Maureen Jenson, MD found no clear correlation between disc abnormalities and back pain.   35% of people with no pain actually have degenerated discs.  It turns out that the discs are not the primary cause of pain and surgically removing all or a portion of the disc does not relieve most pain, is dangerous and a colossal waste of money.   

Many physicians and lay people believe that “pulled muscles” and “slipped discs” are the primary reasons for back pain, ignoring the role that spinal joints and spinal mechanics play in causing and perpetuating back pain. 

Back pain is primarily a physiological issue due to joint dysfunction, spinal misalignments, loss of flexibility, muscle weakness, and compressive forces.  Back pain is not a primary anatomical issue of disc degeneration, arthritis and bone spurs.

A chiropractor is able to safely and effectively with a high patient satisfaction rate, work with patients to quickly improve their joint dysfunction, flexibility, muscle weakness, and lessen the compressive forces of faulty spine mechanics.  This strategy quickly reduces pain, impairment, and increases mobility and strength.  Research is proving that the strategy chiropractors have been using for over a century is the correct one.

For much of chiropractic’s history, one of many reasons medical doctors have derided chiropractic is that they insisted that “cracking the spine” could not possibly influence immunity.  Chiropractors cited many case studies, but each was derided as bias and too small a sample.  Chiropractic was labeled as quacks and from the 1960s though the 1980s the American Medical Association sought to eliminate the profession of chiropractic as unscientific and dangerous.  The spine has absolutely nothing to do with immunity.  The arrogance was astounding.

Now, science is beginning to show that chiropractic does positively influence immunity.  The human immune system is huge and complex system.  The below study compared the rate of antibody synthesis between a group of subjects just adjusted and a control group of subjects not adjusted. 

Researchers separated 76 asymptomatic subjects into two groups.  One group received an upper back adjustment, the control group did not.   Blood was taken before the adjustment, 20 minutes after and 2 hours after.  A bacterial protein was introduced to each blood sample.  As expected, each blood sample showed increased anti-body formation, but the chiropractic group’s blood sample showed a significant increase in antibodies.  This means that the chiropractic adjustment demonstrated improved immunity against a potential infection.  The potential of this study is staggering.   Adjusting did influence immunity, but many questions arise.  Is the increase in immunity short lived and by what quantity.  More research needs to be done, but the outlook is good.

Pain relief is the primary reason why most people first visit a chiropractor.  If each spinal joint doe not move fully and does not move enough, spinal decay, inflammation, and muscle weakness and spasm occurs.  This biomechanical fault causes a postural shift and muscular coordination changes so that the body can better compensate.  These changes then become the new postural pattern.

 Overtime, the muscles surrounding the “stuck” spinal joint continue to work hard in an attempt to better move the joint resulting in muscle spasm, strain and pain.   The tight muscle and tendon start to breakdown and scar tissue and adhesions infiltrate.  This causes further pain and muscle atrophy.

 The chiropractic adjustment breaks up scar tissue and adhesions that were built up in the spinal joints.  The adjustment immediately improves spinal motion.  Unfortunately, the muscles that move the spine often remain tight due to muscle scarring, adhesions and postural habit.

Chiropractic is not known to help people with hypertension, but a pilot study conducted at the University of Chicago has shown that chiropractic can significant reduce blood pressure in people with hypertension.  The study was published in the online Journal of Human Hypertension, on March 2, 2007.

According to lead author George Bakris,k MD, who is the director of the hypertension center at the University of Chicago Medial Center, reports have correlated a rise in blood pressure with neck pain for years.  Dr Bakris, his team, and a lead chiropractor Marshall Dikholtz, DC, decided to test if such a correlation exists.

The Study:

Half the study’s population of 50 hypertensive patients received a chiropractic adjustment and the other half received a fake adjustment.  Blood pressure was taken after the first adjustment and eight weeks later.

 The Results:

The results showed that patients who received the real adjustment saw an average of 14 mm Hg drop in systolic blood pressure and an average 8mm Hg greater drop in diastolic blood pressure

 Patients who received sham adjustment demonstrated no benefit

 The Conclusion:

Chiropractic was shown to significantly lower blood pressure in hypertensive patients because misalignments in the upper cervical (neck) region negatively effects normal blood pressure.

 Why?

There are several possible explanations of why chiropractic lowered the blood pressure of the study’s hypertensive patients.

 1)                  Pain has been shown to increase blood pressure.  Chiropractic has been safely utilized for over a century to decrease neck pain.  Simply alleviating cervical (neck) pain could have been the reason for the drop of blood pressure.

2)                  Nociceptors (nerve endings) originating in the joints of the spine fire pain signals when the spine is not moving properly.  A misaligned spine stimulates the nociceptors, which in turn stimulate the sympathetic nervous system.  The stimulation of the sympathetic nervous system raises blood pressure.  Through chiropractic adjustments, spinal motion improves, nociceptor activity reduces, and blood pressure drops.

3)                  Some proprioceptors (nerve endings) originating in the joints of the spine carry position sense signals to the brain.  When a spine is misaligned and stiff which is common with people suffering from neck pain, the proprioceptor signals become imbalanced.  Since proprioceptor signals inhibit nociceptor(pain) signals, imbalanced proprioceptor signals do not fully blunt the flow of pain signals.  The increased volume of pain signals can cause of rise in blood pressure.  Chiropractic adjustments normalize spinal motion, normalize proprioceptor firing, decrease the flow of pain signals, and decrease neck pain.

According to Dr. Simeon Margolis of Johns Hopkins, “back pain sends more people to the doctor than any complaint except upper respiratory symptoms.”  Unfortunately this common ailment that disables so many is often treated very poorly.   Medicine treats the pain symptoms while totally ignoring the cause.   It would be tragic if ignoring the cause would only result in a rise in the incidence of the leading diagnosis of back pain: muscle sprain, muscle spasm, osteoarthritis, herniated disc, osteoporosis, and fibromyalgia.  But ignoring the cause also results in an increase in heart disease, asthma, diabetes, obesity, high blood pressure, allergies, and depression.  This is because the cause of  chronic back pain and the many chronic diseases are the same.

Back pain is a life-style ailment.  Our life-style is the major cause and perpetrator.  If we choose to follow the medical model of treating back pain as a disease in which we had no responsibility in causing, we may temporarily feel better, but our back and health will premature age.  This is because the same poor life-style that caused back pain is also the primary cause of heart disease, asthma, diabetes, obesity, high blood pressure, allergies, and depression.  Taking care of your back by improving your life-style will dramatically improve your health and how you feel.

Listed below are some life-style choices that lead to back pain and other life-style diseases.

1)         Sedentary life-style.   Not moving enough increases inflammation, causes plaque or fatty deposits to accumulate in blood vessels choking off blood supply to muscles and vertebral discs, starves the discs of nourishment causing vertebral disc disease and osteoarthritis, causes osteoporosis, allows adhesions and scarring to infiltrate spinal joints causing stiffness, inflexibility, and muscle spasms, and increases stress hormones which cause pain sensitivity to rise and immune system levels to sink.

2)         High intake of refined carbohydrates.  Eating high carbohydrate junk food raises insulin and blood sugar levels which delay healing, increases sensitivity to pain, increases inflammation, interferes with collagen repair, narrows arteries, robs spine and other vital organs of essential nutrients, causes muscle spasms, causes osteoporosis, and increases body stress.

3)         Not enough sleep.  Not sleeping enough hours per night delays back healing, decreases muscle coordination (weakens muscle-skeletal system), increases body stress, and increases sensitivity to pain.

4)         Not enough loving relationships.  Not emoting to loved ones causes increase levels of blood stress hormones and pain.

The medical model of treating back pain with rest, medication, advanced imaging, and a short bout of physical therapy is expensive, ineffective, and dangerous.
